STUDENTS GRANT APPLICATION CHECKLIST
Do podania należy załączyć Enclose the following information with your application
Załączyć dowód rejestracji na rok akademicki - student jest przynajmniej na drugim roku studiów
Proof of registration for academic year - you must be at least in your 2nd year of studies
Załączyć wyniki w nauce z roku poprzedniego
Grades from previous academic year
Załączyć opinię prezesa Okręgu KPK lub kuratora Funduszu Wieczystego Milenium względnie proboszcza miejscowej parafii
Include references from branch president CPC, or trustee or parish priest where there is no CPC organization
Kopię dochodu (Notice of Assessment of Income Tax) z roku poprzedniego osoby starającej się o stypendium oraz osób wymienionych w podaniu w 3-ciej sekcji.
Copies of income and Notice of Assessment of Income Tax for previous year of the applicant and other persons (parents/guardians/spouse - where applicable) as shown in Section III in this application.
Podania które nie będą całkowicie wypełnione i/lub bez koniecznych dokumentów nie będą rozpatrywane.
Incomplete applications and applications without the required supporting documents will be rejected.
Podania które wpłyną po 15 września nie będą rozpatrywane.
Please note that the cut off date for applications is September 15th. Applications received at the Canadian Polish Millennium Fund after the above date are deemed ineligible for consideration of student grants.

All correspondence will be addressed to the applicant's home address. In the event of an address change, please inform the Foundation.
